The Wetlands Protection Act prohibits the removal, dredging, filling, or altering of wetlands without a permit. A Notice of Intent is required to obtain such a permit.
The Request for Determination of Applicability provides applicants with the option of seeking a determination whether the Wetlands Protection Act applies to a proposed site or activity.
This form provides a procedure for an applicant to confirm the delineation of a bordering vegetated wetland, and also serves as an application for simplified review of projects located in a buffer zone.
This form is for use by Conservation Commissions to authorize the extension of a previously issued Order of Conditions, for no more than three years.
Upon completion of work authorized in an Order of Conditions, the property owner must request a Certificate of Compliance from the Conservation Commission) stating that the work has been satisfactorily completed.
A notification to be sent to all abutting property owners that a Notice of Intent or ANRAD has been filed with the Conservation Commission for the municipality of Middleton seeking permission or information leading to the removal, filling, dredging, or altering an area subject to protection under the Wetlands Protection Act.
