    As of May 1, 2004, all mailing address changes filed with the Assessors’ Office must be received on this form. Telephone changes and unofficial notes are not acceptable. The policy of the Board of Assessors is to mail all tax bills to the owner of record; not to lenders, management companies, or other institutions. This is to ensure that all taxpayers receive their bill. Please fill out this form legibly and return to the Board of Assessors.
